Mō te tūranga mahi - About the role:
Whanau Ora Navigator The purpose of this role is to work within the Comprehensive Primary and Community Team to support comprehensive, seamless, primary and community health care.
Your priority objectives will be to:
· Improve equity of access and health outcomes for Maori, Pacific Peoples, Tangata
whaikaha, and people living in rural and highly deprived areas
· Access, continuity, comprehensiveness and coordination of care
· Deliver services and supports to enhance the wellbeing of whanau, including having a focus on health, Manaaki or wellbeing
· Focus on a seamless experience of care and support for the person and their whanau
· Navigate across te ao Maori (the Maori world) and te ao Tauiwi (the non-maori world) to understand the Hauora of a tangata (person)
· Represent all people within the health and disability sectors who support tangata (people) to live well, embrace and exercise tino-rangatiratanga (self determination) in navigating their own journey to pae ora, a healthy future.
Mōu - About you You
will be a confident and capable communicator, both written and oral, at ease in both whānau settings and interactions with relevant stake holders. Able to establish rapport with people of all ages quickly and effectively, with proven ability to prioritise and meet deadlines. Your skills and experience will include:
· A positive and encouraging attitude towards helping others
· The ability to communicate effectively both written and verbally
· A team player with the ability to interact with a diverse range of people
· Experience working in a health environment or community setting would be an advantage
· Ability to work independently and with a team
· Ability to speak te reo Māori or other languages would be an advantage
· NZ Full clean driver's license
· The ability to maintain confidentiality and discretion in all work and professional relationships
· Inclusive of, and responsive to, tikanga and Manaakitanga on the marae
